Do you take insurance? As a Direct Primary Care(DPC) practice, we do not accept insurance. We will provide you with the necessary paperwork to submit to your insurance for all visits you attend with Dr. Maidenberg. If you have a PPO, reimbursement will be according to your out-of-network benefit schedule. Also, expenses paid to Sage Pediatrics are typically eligible for reimbursement by your Health Savings Account or Flexible Spending Accounts. How much will my insurance reimburse? Each plan is unique. Therefore, we recommend that you call your insurer and use this form to help determine your coverage. How much time will I have with the doctor? We schedule 60-90 minutes for well-child check ups and other consultations. This allows for a complete physical exam and thorough discussion of your child's health care needs. For sick visits, the appointments are generally 30 minutes long. Are there forms to fill out prior to my visit? Yes, if you are a first-time patient, please fill out the forms below: Intake Form Release of Medical Records				What does a Sage Pediatrics membership include? We are a membership-based Direct Primary Care(DPC) practice. Benefits include: All well-child and newborn visits Direct physician access 24/7 by phone and email Relaxed and thorough office visits (30-90 minutes) Sick visits (same day or next day as needed) Lab orders and review of results Phone, email and video consultations In-office laboratory tests (urinalysis, rapid strep/flu/covid testing) School/camp/physical forms and return to school letters Comprehensive care (coordination with specialists, follow-up visits, chart review, etc) Gender Affirming Care Access to Fullscript Online Supplements Parent counseling/coaching for nutrition, behavior, and sleep Membership fees vary based on the age of the child and the number of siblings enrolled. Please call or email us for pricing for your family. One time consultations such as a Vaccine Scheduling Visit or a Joint Pediatrician/Naturopathic visit do not require membership. Can my child visit Dr. Maidenberg without a membership? Yes, for certain types of visits. We offer two very unique one time consultations: Joint Pediatrician/Naturopathic visit with Dr. Hillary Roland and Dr. Maidenberg($500/700 for 60/90 min visits) Vaccine Scheduling Visit($300 for 45-60 min) These are available for anyone on a fee-for-service basis and do not require membership. Please call or email us to schedule. Does Dr. Maidenberg offer immunizations or newborn hospital care? Yes and yes. There are a few other options that members may choose to add as needed: Vaccines. Available at Sage Pediatrics(at variable costs) or with a collaborating pediatric practice in Berkeley(with your insurance)...call for details Newborn Hospital Care: $450 for one visit or $650 for 2 visits(if delivering at Alta Bates) Home Visit Travel Fee: $100 Integrative Visits with Dr. Maidenberg MD and Dr. Roland ND: $500 for one hour, $700 for 90min. These are Awesome!!! Vaccine Scheduling Visit: $300 for 45-60 minutes in person or virtual ADHD/ED/Depression/Anxiety management(variable prices depending on the case) Do you offer a "Meet the Doctor" visit? Yes!
