Do you take insurance? As a Direct Primary Care(DPC) practice, we do not accept insurance. We will provide you with the necessary paperwork to submit to your insurance for all visits you attend with Dr. Maidenberg. If you have a PPO, reimbursement will be according to your out-of-network benefit schedule. Also, expenses paid to Sage Pediatrics are typically eligible for reimbursement by your Health Savings Account or Flexible Spending Accounts. How much will my insurance reimburse? Each plan is unique. Therefore, we recommend that you call your insurer and use this form to help determine your coverage. How much time will I have with the doctor? We schedule 60-90 minutes for well-child check ups and other consultations. This allows for a complete physical exam and thorough discussion of your child's health care needs. For sick visits, the appointments are generally 30 minutes long. Are there forms to fill out prior to my visit? Yes, if you are a first-time patient, please fill out the forms below: Intake Form Release of Medical Records |
